It is
extremely important that emergency cards be updated each
school year and throughout the school year. These should
also include any person who is allowed to pick up your
student from school in the event we cannot contact you in an
emergency.
If
your student has an emergency care plan, please be sure to
have your doctor update an
“Emergency
Health Care Plan”
form for the nurse so we are aware of any emergency issue
involving your student.
Any
medication your student is required to take during school
hours requires a form be filled out. This form
“Authorization
for School Personnel to Administer Prescription Medication
to Student”
must be completed by the student’s
parent/guardian and the doctor must sign and state how the
medication is to be taken. The medication must be clearly
marked in its original container.
