VOLUSIA COUNTY SCHOOLS

DRIVER EDUCATION INSTRUCTORS

All teachers are State Endorsed in Driver Education and are licensed by the Department of Highway Safety as a Third Party Tester. 

ELIGIBILITY REQUIREMENTS

  1. Student must be currently enrolled in a Volusia County Public High School or School of Choice
  2. Student must be at least 15 years old on or before the first day of the session
  3. Student must have a Florida restricted license/learner’s permit before the first day of the session

HOW TO REGISTER

  1. Student obtains registration form from school guidance counselor
  2. Student takes completed registration form with required parent signature to school registrar
  3. Student is enrolled in course or placed on waiting list

ADMINISTRATIVE GUIDELINES
For a Class Change


Deadline for schedule change request -June 1st

Requests for schedule changes must meet one of the following criteria:

• You are a senior and need course to graduate.
• You have already taken the course and passed.
• Course needed but not on schedule.
• Scheduled for a course that you did not select.

These are the only reasons schedules will be considered……
(You can view your course selections on the parent portal.)


 New Student Registration

1.   Proof of Address

Current utility bill (electric bill, phone bill, etc.), lease or mortgage agreement. If living with a relative or friend due to certain circumstances, then a Residence Affidavit must be signed by the parent/guardian and home owner in the presence of a notary.

2.  Florida Physical Form

If the student has never been enrolled in a Florida School then a physical is needed. If you are an out of state transfer and you have a physical less then 1 year old you can take it to the Florida Health Department for approval. (274-0500)

3.  Shot Records

Immunization history is needed on a Florida Blue Card for entry. Any incoming 9th, 10th, 11th, and 12th grade students are required to have: Hepatitis B series, Tetanus, Diphtheria Booster and a 2nd MMR.

4.  High School Records

In need of withdrawal Papers if coming from within Volusia County. Outside of Volusia County, records must be received (can be faxed) PRIOR to registration. A Records Release form must be signed by the parent in order to obtain such records. Copy of Report Card if available. Any and all ESE records including IEP.

5.  Birth Certificate

Optional

6.   Picture I.D.

Need to make a photo copy of ID from person entering the student.

7.   Student’s Social Security number

8.    Teacher Recommendations

Incoming 9th graders.

9.   Out of Zone

There must be an approved variance in order to enroll. If you are not in zone and wanting to attend Seabreeze then you will need to go to your zoned school and request a variance, once you have the variance then it needs to be brought back to the main office for approval.

Registration Requirements for Transfers

Transferring from another Volusia County School

  • Proof of residency (utility bill, phone bill, rent or lease agreement)

  • Copy of student’s last report card

  • Withdrawal form if transferring during the current school year

Transferring from any other Florida School (public or private)

  • Proof of residency (utility bill, phone bill, rent or lease agreement)

  • Copy of students last report card and transfer grades

  • ESE records including IEP (if applicable)

  • Current shot records

  • Current physical

  • Birth certificate (optional)

Transferring from an out of state school

  • Proof of residency (utility bill, phone bill, rent or lease agreement)

  • Copy of students last report card and transfer grades

  • ESE records including IEP (if applicable)

  • Current shot records

  • Current physical (must be no more then a year old)

  • Birth certificate (optional)

 The items above must be obtained prior to registration. You may call guidance at 386-258-4674 ext. 54606 to schedule an appointment or if you have any questions regarding the registration procedure.

 

Transcript Request Procedure 

  • Currently Enrolled Students = electronic= FREE, paper = $1

  • Graduating Students prior to the last 3 years- Contact Judy Valentino at 386-734-7190 ext. 20433 or through mail at:

Former students can download our Transcript Request Form, once completed you may mail the form with payment to the address below or come in and drop the form off in Guidance from 8:00am to 2:00pm Monday through Friday.

All transcripts include the following information on them:

  • General Student information

  • Immunization information

  • Academic History

  • Diploma Type

  • Total Credits

  • Class/State Rank

  • GPA

  • Test Scores

PLEASE NOTE THAT ALL REQUESTS REQUIRE TWO (2) BUSINESS DAYS TO PROCESS

 

Withdrawals

The withdrawal process starts in Guidance where the parent/guardian of the student will need to come in and have with them:

          *Driver’s license (photo copy needed)  

*Text books or media books (to be returned to the media center or   bookroom)    

Once the paperwork has been completed in Guidance then you are to go to the Media Center/Bookroom and return any and all text books or media books. Once the books have been returned you will then need to go to the Attendance office and see Mrs. Jones who will complete the withdrawal. At that time Mrs. Jones will give you withdrawal papers, these papers will be sufficient enough to get you started at the school where you are transferring to. The new school will fax us a request for further records and once that is received we will send all necessary documents.

Withdrawals need to be done in person through Guidance due to proper verification and documentation.
