To receive your 2nd semester off campus lunch ID, a NEW permission form must be signed by a parent or guardian, turned into the activities office, and pay $10 on or before Wednesday, Dec 18th. If you do not take care of this prior to the holidays you will not be able to go off campus for lunch when your return from break. Pick up a permission form from the Activities office, Student Resources or download the form from the school’s website. Permission forms for 2nd semester off campus lunch ID’s are available to ONLY “eligible” sophomores, juniors and seniors. NOTE: to be eligible, a student must not have 10 or more tardies or absences combined for 5th period from the first semester.
